Contribute Health Communication Materials

You are invited to send materials for inclusion in the M/MC. These will be catalogued and shared with your colleagues worldwide.

HARD COPY BY MAIL

Do the following:

1. Print out the CONTRIBUTION FORM.

2. Complete the CONTRIBUTION FORM for each item. Please note the form requires a signature for permission to share the item.

3. Attach all evidence of the effectiveness of the material.

4. Send the material (packaged carefully) along with the completed CONTRIBUTION FORM to:
